Application Form

• 請依欄位說明正確填寫。

• 請上傳最近 3 個月內脫帽半身正面之彩色大頭照。
  Upload your recent 2 inches x 2 inches bust photograph （taken within 3 months）

護照

Passport

• 無護照者，請上傳國籍身分證件。
  If applicants do not have a passport, you are required to upload your nationality identification document.
• 申請人如有居留證，請務必上傳。
  If applicants have a residence permit(ARC), make sure to upload it.

最高學歷或同等學力證明文件

Certificate of Highest Education Level or Equivalent Qualification

• 畢業證書如非中文或英文，需附上經公證之中文 或英文譯本。
  If the diploma certificate is not in Chinese or English, applicants should submit a notarized copy with a Chinese or English translation.
• 應屆畢業生請上傳在學證明。 Applicants graduating in the current semester are required to upload the certificate of enrollment issued by the current school

歷年成績單

Transcript of Academic Records

歷年成績單如非中文或英文，需附上經公證之中 文或英文譯本。
If the transcripts are not in Chinese or English, applicants should submit a notarized copy with a Chinese or English translation.

財力證明

Financial Statement

• 由金融機構提出足夠在臺就學之財力證明(須達美 金 3,000 元以上)，或提供政府、本校或民間機構 提供全額獎助學金之證明(所獲獎學金總額需達美 金 3,000 元)。
  A financial statement indicating sufficient funds(at least over USD $ 3,000) for studying in Taiwan should be provided, or proof of full scholarships provided by the government, universities (MCUT), or private organizations can be provided. The total amount of the scholarship received must be at least USD $3,000.
• 錄取生若獲其他機構之財力補助，須提供相關證 明文件。
  Admitted students who receive support from other funding institutions, must provide supporting documents.
• 若銀行帳戶名為非本人帳戶，則須出具帳戶所有 人願意資助申請人在台就學花費之切結書。
  If the bank account name stated in the bank statement is not the name of the applicant, a declaration of financial support written by the account owner is required. In that statement, the sponsor must declare that she or he would pay for all expenses incurred during the applicant’s studies in Taiwan.
• 財力證明不得採用代辦、公司行號或個人開立證 明。
  Financial Statements cannot be provided by agencies, companies, or individuals on behalf of the applicant.
• 申請中之各類獎學金申請表不能作為財力證明。
  Any kind of scholarship application form or scholarship still in the application procedure cannot be used as a financial statement.

語言能力證明

Language Proficiency Certificate

學士班 Bachelor

• 申 請 者 需 附 華 語 文 能 力 測 驗 漢 語 水 平 考 試 （HSK）至少四級（Level 4）、華語文能力測驗 （TOCFL）至少(Level 2）或其他達 A2 級之中文 檢定證明。
  Applicants are required to Chinese proficiency certificate (New HSK level 4 or TOCFL level 2 or relevant certification above CEFR level A2).
• 如符合以下條件，可提供相關證明，不須再繳交 語言測驗證明。
  Applicants applying to a program conducted in Chinese (including Chinese-taught programs and programs with sufficient Chinese courses to satisfy graduation requirements) do not need to provide a certificate of language proficiency if:
  • 母語為中文者，請提供中文自述說明。
    If Chinese is your native language, you can provide an explanation in Chinese to show your language ability.
  • 前一學位主修為中文者，請提供畢業證書作為 佐證資料。
    If your previous degree was majoring in Chinese. You can use your diploma as proof.
  • 先前任一學位(含高中畢業證書)為全中文授 課，請提供學校開具的證明文件。
    If any of your previous degrees (including senior high school diploma) were entirely taught in Chinese, we will only require that you submit a certification provided by your previous university/high school.

博碩士班 PhD & Master

• 申請者需附英文能力檢定證明至少 CEFR B1。
  Applicants are required to provide an English proficiency certificate CEFR B1 or higher.
• 如符合以下條件，可提供相關證明，不須再繳交 語言測驗證明。
  Applicants applying to a program conducted in English (including English-taught programs and programs with sufficient English courses to satisfy graduation requirements) do not need to provide a certificate of language proficiency if:
  • 國籍為英語系國家，則無需出具任何文件。
    Applicants from English-speaking countries are not required to submit any documents.
  • 先前任一學位(含高中畢業證書)在英語系國家 取得，請提供您的畢業證書作為證明。
    If any of your previous degrees (including senior high school diploma) were from an English-speaking country, we will only require that you submit your graduation certificate. No further proof of English language proficiency will be required.
  • 先前任一學位(含高中畢業證書)為全英語授 課，請提供學校開具的證明文件。
    If any of your previous degrees (including senior high school diploma) were entirely taught in English, we will only require that you submit a certification provided by your previous university/high school.
  • 英語系國家請參考簡章附件三「官方語言及通用 語言為英語之國家名單一覽表」。
    For English-speaking countries, please refer to Appendix Ⅲ “The List of Nations with English as the Official or Common Language”.

讀書計畫

Study Plan

• 申請學士班者請以中文撰寫。
  Bachelor applicants are required to write in Chinese.
• 申請博、碩班者請以英文撰寫。
  PhD and Master applicants are required to write in English.
• 內容可包括選擇本校或系所的動機以及就讀本校 時的短、中、長程學習計畫。
  The content may include motivation for choosing MCUT and the short, medium, or long-term study plan in MCUT.
